The home health nursing position offers the opportunity to provide care flexibly through a reliable and trusted community hospital.

Responsibilities of the RN Nurse Home Health, Home Care

• Collaborate with the larger medical team, including dietitians, physical therapists, social workers, etc.
• Ensure patients are following treatment plans
• Practice safe and compliant nursing care
• Work independently in patients’ homes and provide evidence-based nursing care
• Strive to restore patients health and minimize ailments and sickness
• Take vitals, draw for labs when necessary
• Assist with patient mobility
• Communicate respectfully with patients and families
• Requirements for the RN Nurse Home Health, Home CareAssociate’s Degree Nursing Required
• BSN Bachelor’s Degree Nursing preferred
• ACLS preferred
• Current RN Nursing license in the state of New York
• Valid NYS Driver’s License
• ICU or Medical-Surgical nursing experience
• Home health nursing experience preferred
