The home care department provides excellent services such as PACE (Elder Care), home health, and hospice. The ideal RN Manager will collaborate with nurse leaders to develop a motivated nursing team that can manage patient-centered care plans while remaining cost-effective.
The Magnet-Designated medical facility provides patients comfortable care in their homes and works to shorten LOS in the hospital. The RN Nurse Manager for Home Health will work with a multi-disciplinary team to implement care plans and services like home rehabilitation, Hospice, managing conditions at home, PERS, and tele-health at home.
The RN Manager, Home Health is responsible for 24/7 nursing management and leads professional practices consistent with the unit’s goals of safety, customer satisfaction, and care quality.
Responsibilities for the Home Care Nurse RN Manager:
24/7 management of region’s Home Health nursing staff
Assist in management evaluation and improvement while reporting to Home Health Director
Interview, hire, train and mentor new and current registered nurses and home health staff
Collaborate with RN Nurse Educators and Education Professionals to develop regular training programs
Develop and evaluate nurse goals and objectives with initial 90-day and annual performance evaluation
Screen home care needs of referrals in the areas of nursing, rehabilitation and psychosocial
Assign and manage an efficient workflow to team in a timely manner
Serve as a liaison between nursing team and with hospital therapists and physicians
Perform announced and unannounced patient visits to ensure both comfort and evidence-based care is being provided
Assist nurses in completing assigned patient care plans with tasks such as patient mobility and administering medications
Utilize the Outcome and Assessment Information Set (OASIS) to evaluate the home health care patient outcomes
Record vital sign alert reports and treatment actions into updated EMR
Use quality assurance performance improvement (QAPI) indicators when reviewing current patient-focused care plans
Evaluate and manage fiscal budgeting including billing/revenue management, payroll, purchasing and supply cost management
