Premier acute-care hospital that improves at-home patients’ health is hiring a strong leader for their Home Health Division. The ideal Nurse Director, Home Health is not only experienced in-home health and nursing management but can organize a cost-efficient workflow to shorten LOS in hospitals and provide patients with accessible and comfortable care in their homes.
The Patient Care RN Nurse Director for Home Health will provide direction to the Operations Manager, Clinical Managers, and nursing staff, who offer a full spectrum of home care services to meet the individual needs of patients. Partnering with patients’ physicians and designated therapists, the home health/home care department will coordinate needed care and treatments from skilled nursing care and infusion therapy to rehabilitative therapies and hospice care.
The Home Care department is backed by a health system nationally recognized for its care excellence. The RN Director will continue to lead patient care beyond expectations. Responsibilities of the RN Nurse Director Home Health/Home Care:
Oversee Clinical Managers, nurses, rehabilitation therapists, medical social workers, and home health aides
Collaborate with the Operations Manager and Clinical Supervisors to evaluate current case management and treatment plans
Report to the Home Health Administrator with Home Health operational and patient changes
Onboard, hire, train, and mentor new and current nurses
Develop and evaluate nurse goals and objectives with initial 90-day and annual performance evaluation
Screen and assess home care needs of referrals in the areas of nursing, rehabilitation, and psychosocial
Assign and manage an efficient workflow to the team in a timely manner
Participate in patient visits to provide both comfort and evidence-based care
Act as a liaison between the nursing team and hospitals, outpatient discharge planning services, and physician offices
Assist nurses in inpatient care as needed, such as patient mobility, completing patient care plans, and compassionate communication with patients and families
Monitor and schedule patient assessment requirements and timelines
Review and process vital sign alert reports and documents action into the EMR system
Complete reviews of current evidence-based care plans using quality assurance performance improvement (QAPI) indicators
Evaluate and manage regional financial and fiscal budgeting
Partner with the Operations Manager to control costs and pursue additional revenue opportunities
Ensures policies and procedures comply with the New York State Dept. of Health regulations and Joint Commission standards
