Award-Winning Virginia Hospital Seeks Home Health Intake Nurse Manager

Join a compassionate nursing team as a Home Health Intake Nurse Manager overseeing new patient intakes and referrals. Coordinate patient needs with an interdisciplinary team of specialists to ensure the highest level of care is given to every patient. An ideal candidate can multitask, stay organized and follow up on each unique patient care plan.


Responsibilities for the Nurse Manager Home Health Referral/Intake:

• Interview potential patients to assess needs
• Work with larger interdisciplinary team of therapists and specialists
• Follow up on patient progress
• Coordinate, screen, and assess home care needs of referrals in the areas of nursing, rehabilitation and psychosocial
• Ensure compliance with state and federal regulations
• Work independently in patients’ homes and provide evidence-based nursing care
• Strive to restore patients health and minimize ailments and sickness
• Take vitals, draw for labs when necessary
• Assist with patient mobility
• Communicate respectfully with patients and families

Responsibilities for the Nurse Manager Home Health Referral/Intake:

• BSN Bachelor’s Degree Nursing
• ACLS preferred
• Current RN Nursing license in the state of Virginia
• Valid Virginia Driver’s License
• ICU experience a plus
• 1+ year experience in acute care nursing
• Home health nursing experience
• Rotating holidays, weekends and on-call
