US News and World Report High Performing Hospital is hiring a Nurse Practitioner to lead program design for their brand new Advanced Illness Management services. The ideal Program Manager uses their Adult and/or Geriatric Nurse Practitioner NP experience in Palliative and Advanced Illness to train and educate AIM providers on the Advanced Illness Program, ensuring high-quality standards of practice. The Advanced Illness Management (AIM) Program Manager, Nurse Practitioner will develop and implement plans within the Palliative Care Unit that assist patients with pain management, infection control, and therapy. The NP Program Manager educates the Palliative care team, including physicians, nurse practitioners, and social workers, on best AIM practices for patients suffering from advanced diseases. The NP must bring a team-focused approach, working on education, training, and growth of the nursing staff.
Responsibilities for the Advanced Illness Management (AIM) Program Manager APRN, Nurse Practitioner :
Responsible for clinical management and education of AIM teams, including physicians, nurse practitioners, social workers
Manage training and education of AIM and Palliative Care, staff
Act as a resource for AIM staff with updated educational training and best practices
Report to the Chief of Palliative Care to support and carryout divisional plans and programs that meet quality metrics regarding Advanced Diseases
Work within the grant-funded program to educate and train physicians on the utilization of the program and reporting
Collect, review and analyze Utilization Review data and report findings to leadership
Ensure appropriate care plans are followed, including pain management, infection control, and therapy
Maintain working knowledge of industry standards related to AIM and palliative care
Requirements for theAdvanced Illness Management (AIM) Program Manager APRN, Nurse Practitioner:
MSN, Masters of Science in Nursing
MUST be a Certified Nurse Practitioner NP
Registered Nurse in the state of NY
2+ years as a Nurse Practitioner in Palliative/Geriatric Care, Advanced Illness Management, or related field required
BLS/CPR Certification
CHPN/ACHPN or associated certifications preferred
2+ years experience in program management, grant work, or related field preferred
Healthcare Project Manager Professional PMP certification is a plus
Experience collaborating with physicians and interdisciplinary teams
Working knowledge of Advanced Illnesses within Palliative Care education techniques
