Responsibilities for the Advanced Illness Management (AIM) Program Manager APRN, Nurse Practitioner :
- Responsible for clinical management and education of AIM teams, including physicians, nurse practitioners, social workers
- Manage training and education of AIM and Palliative Care, staff
- Act as a resource for AIM staff with updated educational training and best practices
- Report to the Chief of Palliative Care to support and carryout divisional plans and programs that meet quality metrics regarding Advanced Diseases
- Work within the grant-funded program to educate and train physicians on the utilization of the program and reporting
- Collect, review and analyze Utilization Review data and report findings to leadership
- Ensure appropriate care plans are followed, including pain management, infection control, and therapy
- Maintain working knowledge of industry standards related to AIM and palliative care
- MSN, Masters of Science in Nursing
- MUST be a Certified Nurse Practitioner NP
- Registered Nurse in the state of NY
- 2+ years as a Nurse Practitioner in Palliative/Geriatric Care, Advanced Illness Management, or related field required
- BLS/CPR Certification
- CHPN/ACHPN or associated certifications preferred
- 2+ years experience in program management, grant work, or related field preferred
- Healthcare Project Manager Professional PMP certification is a plus
- Experience collaborating with physicians and interdisciplinary teams
- Working knowledge of Advanced Illnesses within Palliative Care education techniques
