The ARRA Group, a boutique healthcare recruitment firm, has partnered with our distinguished Tennessee hospital client to hire a patient-oriented, empathetic RN Case Manager for Inpatient Acute Care.
The ideal Nurse and Case Manager is an excellent communicator with strong organizational and problem-solving skills. This Case Manager will work with a strong team committed to patient-centered care.
Responsibilities

Conduct assessments to identify the patient's medical, social, and financial needs

Collaborate with the patient, their family, and the healthcare team to develop a comprehensive care plan that meets the patient's individual needs and goals

Develop a post-hospital care plan that includes referrals to other healthcare providers, such as home health agencies or rehabilitation facilities

Ensure that the patient's medical record is accurate and up-to-date and that all documentation is completed in a timely manner.
Requirements

Bachelor's Degree in Nursing required

Current RN Nurse Licensure in the State of Tennessee

Strong case management experience within acute inpatient care settings
