Position: LMSW/LAPSW Case Manager, Inpatient Acute Care Employment Type: Full Time Location:Memphis, TN Salary: $47,507-$62,899 + Full Benefits
The ARRA Group, a boutique healthcare recruitment firm, has partnered with our highly-regarded Tennessee hospital client to hire a compassionate and organized Inpatient, Acute Care, Social Work Case Manager to assist in pre- and post-discharge client care plans.
The LMSW or LAPSW Case Manager will collaborate with a strong Case Management team and other agencies and available community resources to assist and support clients and families on health, safety, and wellness topics.
Responsibilities
Conduct assessments of client's needs and develop individualized care plans using resources to identify the best services and support
Provide and coordinate direct services to clients, including counseling, advocacy, crisis intervention, and referrals to community resources
Maintain accurate and up-to-date client records, including progress notes, care plans, and other documentation required by funding sources
Provide comprehensive clinical social work and discharge planning to patients and families
Advocate for the safe discharge of the patient to home or community resource and supports preventive and follow-up care post-discharge
Participate in team meetings and collaborate with colleagues to ensure clients receive high-quality, coordinated care
Requirements
Master's degree from an accredited School of Social Work
Tennessee license of Social Work such as LMSW or LAPSW
Case management experience within acute inpatient care settings
